What Is the SCN Program

NY Medicaid's community-based referral network.

New York's Social Care Network (SCN) program connects Medicaid members to community-based organizations (CBOs) for social determinant-of-health services — housing, food, transportation, and more. CBOs entering the network must complete a structured onboarding process to satisfy OMIG, DOH, and HIPAA requirements before going live.
